Retiring Benefits Request
Attachments:
1. Application for Pension/Gratuity form. (This form is required to be completed by all officers applying for benefits under the Pensions Act).
2. Birth certificate (original or certificated copy required).
3. Letter and proof of date of appointment to the Public Service. (For Police Officers copy of Attestation Paper).
4. Periods of leave without (unpaid leave).
5. National Insurance Number.
6. Employee Number.
7. Request for retirement benefit form (This form is to be completed by the Human Resource Officer).
8. Salary progression.
Instructions:
1.A completed application for retirement benefit filled out and signed by the submitting officer

Instructions:

Unpaid Leave
Instructions:
1.The officer must submit a letter of request and supporting documents (if necessary) to immediate supervisor.
2.The supervisor will forward the letter of request to HR who in turn submits the request to PS/Head of Department for approval.
3.Unpaid Leave may be rejected at this stage based on the needs the agency at the time.
4.Once approved by the PS the HR section must then make a recommendation to the Department of Public Service.
5.The Permanent Secretary of the Department of Public Service must approve the request which is then returned to the Agency who will advise the officer accordingly.
